Through the National Defense Authorization Act (NDAA) of 2018, Congress authorized DOD in 2028 to adjust additional drug payments to reflect changes in pharmacy program costs. It is feared that the Health and Safety Authority may use this agency to implement excessive revenue increases.
In the coming year, he will work to achieve legal hurdles at this agency based on previous laws governing TRICARE pharmacy benefits.
The FY 2001 TRICARE For Life NDAA Act expanded TRICARE’s home delivery and pharmacy programs to Medicare-eligibles who were previously limited to filling prescriptions at military medical facilities (MTF).
The first TRICARE pharmacy payments established by the Department of Defense followed the intent of the law and conformed to business model standards. Since then, Congress has enacted higher dispensary fees.

Recently, Congress included a two-year schedule for increasing pharmacy payments through 2027 in the 2018 NDAA. These increase TRICARE payment levels with other employer-sponsored plans.
The Kaiser Family Foundation (KFF) Employer Health Benefits Survey reported average drug reimbursements of $11, $37, and $67 for Tier 1, Tier 2, and Tier 3 drugs, respectively. TRICARE pharmacy costs $14 for prescription drugs, $38 for brand-name drugs and $68 for non-prescription drugs for a 30-day supply.
A 90-day supply by mail order costs $12, $34, and $68 for regular and non-regular types, respectively. Prescription drugs available at MTF pharmacies do not have cost sharing.
Unlike most commercial plans, TRICARE does not have a certain level of prescription drugs, such as biologics, that can be used to treat chronic conditions or certain cancers. Specialty drugs can be expensive and often require special care, administration (eg by injection or infusion) and patient monitoring.

According to KFF reports, about 61% of employees at large companies have a health plan with some level of cost-sharing only for specialty drugs. The average copay for specialty drugs in these plans is $103 and the average cost sharing is 25%.
The TRICARE pharmacy program does not have a separate, more expensive line of specialty drugs, but TRICARE worries about the future of pharmacy payments.
